                                       Animal Care Contract

PLEASE READ THE FOLLOWING STATEMENTS AND INITIAL TO INDICATE YOUR UNDERSTANDING AND AGREEMENT TO TERMS OF ADOPTION THROUGH ANGELPAWS animal rescue society.

____  Do you understand that adopting a pet is lifelong commitment and that dogs can live 10-20yrs.

____ Do you understand that you are not allowed to surrender your dog to the pound or shelter, if you are no longer able to keep your pet you are to contact ANGELPAWS before you rehome them.

____Do you understand that ANGELPAWS has the right to home visits and phone check ups, if at anytime your animal is not being properly taken care of ANGELPAWS has the right  and moral responsibility to remove and rehome that animal.

____I agree my dog will wear current ID at all times including a current rabies tag.

____I agree my dog will be current on all vaccinations and receive regular veterinarian care as needed.

____ I agree my dog will be provided with quality dog food, fresh water and appropriate shelter at all times and be treated with love, attention and care.

____I agree that my dog will never be constrained by a rope, cable or chain at anytime, but will have an adequate size fenced in yard and receive walks, playtime and exercise.

____I agree that if I must leave my dog for more than one day, they will be boarded in a kennel or provided care by a reliable caretaker.

____I understand that to the best of their ability animals are monitored for behavior, health and personality by ANGELPAWS, and that they will share any issues of which they are aware but are not responsible for conditions/behavior of which they had no knowledge.

____I understand adoption fees help cover some of the cost of care the animal has received. This fee is considered a donation and non-refundable.

____I understand that ANGELPAWS will remain available to me for questions, problem solving or any issues I may have concerning my dog even after adoption.

My signature below testifies that I have read and agree with the terms of this legal contract for adoption of a rescued animal through ANGELPAWS animal rescue society.
